With the intensification of global population aging, the incidence of neurodegenerative diseases (NDs) is continuously rising. This type of disease is characterized by progressive loss of neuron groups, with cognitive decline and motor dysfunction as typical symptoms. For the time being, drugs for NDs mainly focus on alleviating symptoms and have limited effect on preventing the progression of the diseases. In recent years, exosomes have gradually become a research hotspot in the treatment of NDs. It has been found that exosomes derived from microglial cells and astrocytes have bidirectional regulation on pathology, and exosomes derived from mesenchymal stem cells have therapeutic potential for NDs. This article will systematically review the structure and properties of exosomes, and state the mechanism and therapies in NDs in virtue of the roles of the above three typical exosomes in the central nervous system in order to provide new treatment ideas.
