Refractory obstructive sleep apnea (OSA), as a difficult subtype in the field of sleep breathing disorders, has always been a key and difficult issue in the clinical management of respiratory and sleep medicine. The limitations of conventional continuous positive airway pressure (CPAP) treatment for this type of patient are becoming increasingly prominent, and there is an urgent need to explore more optimized combination therapy options in clinical practice. This article focuses on the treatment pathway of inhaled nitric oxide (iNO) combined with CPAP, and conducts a systematic analysis from four aspects: pathological and physiological mechanism compatibility, clinical efficacy, safety performance, and clinical application value. The aim is to clarify the logic and application prospects of the combined treatment plan for refractory OSA, provide theoretical reference for optimizing clinical treatment decisions, and provide ideological support for the improvement of relevant diagnosis and treatment standards in the future.
